When this privacy notice applies

This privacy notice applies to your use of the Aged Care Assessor App.

This privacy notice explains how we (the Australian Government Department of Health and Aged Care) manage personal information to meet our obligations under the Privacy Act 1988 (Cth) and the Australian Privacy Principles.

Information we collect

We collect the following personal information about you:

• your name 
• your email address
• your activation code (obtained through the Assessor Portal)
• your Assessor ID
• your organisation and role details
• your device details (make, model, operating system, App Store used for download)
• system logs when you use the App.

Why we collect this information

We use your personal information to:

• authenticate your identity when you access the App
• uniquely identify and monitor you as an authorised user of the App
• assist you to access and use the App if you contact our My Aged Care service provider and assessor helpline. 

We require this information to ensure you are authorised to access the App.

You do not have to provide your personal information to us. But you will not be able to access the App if you do not provide your personal information.

How we collect this information

We collect your information:

• directly from you when you authenticate in the App after you have downloaded it to your device
• when you access the App
• in system logs, when you use the App.

We may also collect your personal information to verify your identity and help you access or use the App if you contact the My Aged Care service provider and assessor helpline.

Who we disclose your personal information to

Your personal information will be visible to relevant staff of the Department of Health and Aged Care.

We will not disclose your personal information to overseas recipients.

How we store your information

We hold your personal information in the My Aged Care system, which is hosted on secure databases in Australia.
